We treat your information with the same care as your medical record. This page explains what we collect, how we use it, and the choices you have.

Information we collect

We collect information you provide directly — for example, when you submit an appointment request, contact our office, or check in for a visit. This typically includes your name, contact information, date of birth, insurance details, and a brief description of your symptoms.

We also receive standard technical data when you use our website, such as IP address, device type, browser, and pages visited. This information is used for normal website operations, security monitoring, and high-level analytics, and is not used to identify you personally outside of our clinical workflow.

If you choose to use the appointment request form, the information you submit is transmitted directly to a staff member at our practice through our email provider. We do not store form submissions in a separate marketing database.

How we use it

Your information is used to schedule and provide care, verify insurance, communicate appointment details, coordinate referrals, and operate our practice. Limited, de-identified usage data may also be used to improve our website and patient communication.

We do not sell your information. We do not use protected health information for advertising or marketing communications without your specific written authorization, and we do not share PHI with social media platforms for ad targeting.

HIPAA & protected health information

Protected health information (PHI) is handled in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable Texas law. Our full Notice of Privacy Practices is provided at your first visit and is available on request from our office. It explains in detail your rights regarding PHI, how we use and disclose it, and how to file a complaint.

Information submitted through our website forms is treated with the same confidentiality as the rest of your medical record. Internet email is not a fully secure form of communication, so please do not include sensitive clinical information in the contact form — once you become a patient, we will direct you to a secure patient portal for any clinical communication.

If you are a patient and need to exercise your rights under HIPAA — including the right to access, amend, or request restrictions on your PHI — contact our office and we will provide the necessary forms and respond within the time frames required by law.

Cookies & analytics

We use a small number of cookies and analytics tools — such as Google Analytics 4 with IP anonymization — to understand how visitors use our website. This data is aggregated and is not used to identify individual patients.

You can control cookies through your browser settings or through the privacy controls offered by the analytics provider. Disabling cookies will not affect your ability to schedule an appointment or receive care.

Third-party services & business associates

We rely on a limited set of trusted vendors for hosting, secure email delivery, scheduling, billing, and analytics. Vendors that have access to protected health information operate under HIPAA-compliant business associate agreements and are bound by their own privacy and security obligations.

We do not share PHI with third parties for their own marketing purposes. We may share PHI for treatment, payment, and health care operations as permitted by HIPAA, or when required by law.

Your rights

You have the right to inspect and obtain a copy of your medical record, request corrections, request a list of certain disclosures we have made, request that we limit how we use or share your information, and request confidential communication at an alternate address or phone number.

Patients in Texas additionally have rights under the Texas Medical Records Privacy Act (HB 300). Contact our office to exercise any of these rights — we will respond within the time frames required by HIPAA and Texas law.

Non-discrimination & accessibility

Interventional Pain Management does not discriminate on the basis of race, color, national origin, age, disability, sex, gender identity, or sexual orientation in the provision of care or in the operation of this website. Language assistance and accessibility accommodations are available on request.

Updates to this policy

We may update this policy from time to time to reflect changes in our practices or in applicable law. The effective date above reflects the most recent revision. Material changes will be highlighted on this page and, where appropriate, communicated through our Notice of Privacy Practices.
